Instructions for Submitting RFC Form

After the form has been completed, submit it electronically to the CIO Group of the UCSF IT Governance Committee at ciogroup@itgovernance.ucsf.edu. Comments should be submitted to the CIO Group of the UCSF IT Governance Committee via their email address.
