SCOPE:

The Request for Comments Form is to be used by the committees, subcommittees, sub-groups, and task forces of the IT Governance Structure to propose an information technology policy, standard, or guideline for adoption and implementation at the enterprise-wide level. This applies to policies, standards, and guidelines that have a direct or indirect effect across all departments.

DEFINITIONS:

  • Policy: A policy is typically a document that outlines specific requirements or rules that must be met.

  • Standard: A standard is typically a collection of system-specific or procedure-specific requirements that must be met.

  • Guideline: A guideline is typically a collection of system-specific or procedure-specific "suggestions" for best practice. They are not requirements to be met but are strongly recommended.

PROCESS STEPS FOR APPROVAL OF PROPOSAL:

  1. The originating committee or subcommittee submits a Request for Comments Form to the CIO Group* of the UCSF IT Governance Committee;

  2. The CIO Group forwards the proposal to the chairs and vice chairs of the IT Governance Committee, subcommittees, subgroups, task forces and any target audience identified on the RFC form, for review and comments. The chairs and vice-chairs share with their committees and others what they consider appropriate;

  3. Comments from the CIO Group, IT Governance Committee, subcommittees, subgroups, task forces, and target audience are sent to the originating committee or subcommittee and are incorporated into or amended to the proposal, which is then returned to the CIO Group;

  4. The CIO Group submits the RFC to the UCSF IT Governance Committee; and

  5. The UCSF IT Governance Committee approves the proposed policy, standard, or guideline, and forwards it to the campus community for implementation.
